This form, provided by the Islamic Religious Council of Singapore (MUIS), is for requesting food verification and/or ritual cleansing services.
Important notes:
1. "Food Verification Only" (FVO) and "Ritual Cleansing Only" (RCO) services are mutually exclusive. If separate requests for FVO and RCO are submitted for the same event, both requests will be rejected and fees paid will not be refunded.
Select "No" if your menu includes high tea, lunch or dinner options.
1 set = dinnerware items (e.g. plate, bowl, cutlery) used by 1 pax. Include extra buffer sets (if any). Example: For 200 pax + 10 buffer sets, enter 210.
Note: For Marina Bay Sands (MBS), due to the size of its kitchens, please declare as follows: Level 3 - Two (2) kitchens; Level 5 - Three (3) kitchens.
1 menu item = 1 unique dish or option. Example: For a lunch menu with 2 appetisers + 3 main courses + 2 desserts, enter 7 (2 + 3 + 2).
If the submission date is less than 14 days from the event start date, there will be additional charges for late submission.

It is strictly for applicants who have submitted an application via the GoBusiness Licensing (GBL) Portal. One request shall be submitted per premises.
